Every Year, many people consider liposuction for body contouring, and most want to know if they are a good candidate for the procedure.  Good candidates for the procedure are men and women who are at or near their ideal body weight, but have areas on their body that are out of proportion to the rest of their body, and seem to be resistant to diet and exercise.  The typical areas for women are the lower abdomen, the outer thighs, inner thighs, hips and arms. For men, the typical areas are the abdomen and love handles, also known as the flanks.  But any area of the body can be troublesome, and hold onto excess fat. Most of this is due to genetics and lifestyle. Many of these other areas, such as the chin, & neck can benefit hugely from liposuction for contouring.    Although many people may not fit into the “ideal” candidate category, it may still be a good option for some.  The best way to know for sure, is to be evaluated by a Board Certified Plastic Surgeon who has experience in Liposuction procedures.  Dr. Amy Bandy has performed liposuction, also known as suction assisted lipectomy, liposculpture, microliposculpture, lipocontour, and body contouring for over 18 years.

 

Liposuction surgery is offered in a variety of techniques.  The traditional standard liposuction that most Plastic & Reconstructive surgeons perform is the tumescent technique, or wet technique.  This procedure starts with wetting the fatty tissues with a mixture of solutions to allow the expansion of the tissues, shrink the blood vessels to reduce the amount of bleeding during surgery, and to allow an even removal of the fat along with the fluids for a smooth result.  In addition to this procedure, the ultrasound can be added to help break up or “melt” the fat prior to removal.  The ultrasound can be used either by introducing an ultrasonic cannulae directly into the fatty tissues, or by applying an ultrasound transducer outside through the skin after the fluid has been injected.  After treatment of the tissues with the ultrasound, the fat is then removed in the same way that using a cannulae to suction it out.  Laser liposuction is another technique, where the laser is introduced into the tissues just under the skin to treat the fat in this layer. The theory is that the laser should be able to help dissolve the superficial layer of fat, coagulate blood vessels to reduce bleeding and bruising, and to stimulate the production of collagen to assist in skin tightening. Another method is the PAL, or power assisted liposuction, which is a vibrating hand held cannulae that oscillates forward and backward to assist the surgeon during the procedure to help reduce surgeon fatigue.  Another recently introduced method is the “Tickle Liposuction” also known as Nutational Infrasonic Liposculpture (N.I.L), which is a whirling nutational infrasonic vibration technique that is meant to confuse the brain regarding the type of sensation it is feeling in hopes that it reduces the interpretation of pain and the brain then thinks it is a sensation of tickling.  This is utilized to allow liposuction under a local anesthesia so that the patient is awake during surgery in hopes that they will have minimal discomfort.

 

Although some of these techniques sound superior than the others, studies have not verified that one specific type of procedure over the other exhibits better surgical outcomes.  In fact, most surgeons who have tried a variety of these procedures will agree that the real determining factor for superior results is the operator who is doing it, in other words, the ability of the surgeon.  Studies have not been convincing enough to show that the specific type of procedure is what is contributing to better results.

 

Many surgeons will utilize some of these adjuncts to the traditional liposuction procedure when they feel that there will be a benefit to help reduce the operation time, or when the tissues that are being treated are significantly fibrous or scarred.  But in short, the best results will be obtained when performed in the best surgeons hands on the most suitable patients.
